Add sender email address to Alert Notifications screen

Currently the CSM GUI configuration for Alert Notifications asks for the "Reply-to-address".  This is not the same as the sender address but this subtle difference is easily missed (and if noticed there is no obvious way to specify the sender address).  The sender address, "defaults to a name built using the server hostname or IP".  In some systems, including mine, the sender domain needs to match client email domain or the address must be a valid user.  Minimally, the GUI should indicate what sender address will be used along with a note that it can be overridden using the server properties configuration file.  Even better would be to display the default sender address but allow the user to override it right in the GUI.
